Discharge Follow-Up Workflow

After a member is discharged, a follow-up call should be completed within 7 days to reduce readmission risk.

Checklist

  • Confirm discharge medications and instructions.
  • Schedule a PCP follow-up appointment.
  • Update the care plan with any new diagnoses.
  • Document the follow-up call in AYCE.
